STUDENT ABSENCE REPORTING

Parents/Guardians are required to complete the "Student Absence" Form to report their child's absence for the day for both PLANNED absensces and ILLNESS.

-Illnesses need to be reported DAILY.
-Planned absences can be reported ONCE with the date range listed in the description box.

You DO NOT need to call or email the Main Office or Health Office.

Health History and Physical - to be submitted before your child/ren will be permitted to attend

• Part A: History - to be completed by parent/guardian and reviewed by examining health care provider noting any special health considerations. Please note any medications to be administered in school and submit Authorization to Administer Rx/OTC Medication Form with MD signature. IF you wish for your child to receive Tylenol, Ibuprofen, or Tums, please submit the Parent/Guardian Authorization to Administer PRN Medications in School Form.
• PHYSICAL EXAM: (bottom section) The physical exam section may be filled out by the physician or a copy of the MD’s form may be attached.  All students must be examined upon entry into the school district.  This exam must be done no more than 365 days prior to entry.  You do not need a new physical exam appointment.  You may have the form completed based on the child’s most recent physical exam given it is within the past 365 days of entry.
• Special Considerations: Allergy, asthma, authorization to administer medications in school and other special health needs forms will also need to be submitted annually. Please find these forms on the school website.

• Preschool Age (2’s-Transitional Kindergarten):

  • Diptheria, Tetanus & Accellular Pertussis (DTap) - 4 doses
  • Polio (IPV) - 3 doses
  • Pneumococcal Conjugate Vaccine (PCV) - at least 1 dose on or after their 1st birthday 
  • Haemophilus Influenzae (HIB) - at least 1 dose on or after their 1st birthday
  • Varicella Vaccine (Chicken Pox) - 1 dose required on or after their 1^st birthday (but 2 doses are recommended)
  • Measles, Mumps, Rubella (MMR) – 1 dose (on or after their 1st birthday)
  • Influenza - one dose annually for children age 6-59 months by December

• Kindergarten Students (in addition to the Preschool requirements):
   
  • Diptheria, Tetanus & Accellular Pertussis (DTap) - 5 dose or 4 doses only if last dose is on or after their 4th birthday
  • Polio (IPV) - 4 doses or 3 doses only if last dose is on or after their 4th birthday
  • Hepatitis B - 3 doses (a completed series)
  • Measles, Mumps, Rubella (MMR) - second dose of MMR

• 6th Grade Requirements (in addition to above):
  • Meningococcal Vaccine - 1 dose 
  • Tetanus Booster (Tdap) - 1 dose

Annual Screenings

As required by law, the health office will be conducting yearly hearing screenings for children in Kindergarten-3rd grade, as well as grades 5 and 7. Annual scoliosis check will be done for grades 5 and 7. If you do not want your child to receive any screenings, the opt out form can be found here and must be returned to the nurse by November 2nd. If the form is not received by this date, your child will be screened in accordance with New Jersey Administrative Code – Chapter 16.

Outdoor recess is an integral part of the school day and provides students with a break from structured classroom time. Recess promotes opportunities for physical exercise and social development. 

Children are expected to come to school prepared to participate in outdoor recess.

When winter weather conditions exist, it is essential that children have the proper clothing to prevent hypothermia. Hypothermia may result if the body’s heat loss is greater than its rate of producing heat. Appropriate outerwear such as a pair gloves or mittens, a warm coat, and hat is essential for healthy and safe outdoor play.  It is suggested that girls wear tights/leggings under their jumpers/kilts to keep their legs warm during outdoor recess during these colder months.
